Resources Heartland Family Service - Heartland Bridges

Offers two services:  Transitional Living, and Crisis Care.  Staff are available 24 hours for resident oversight and assistance.  Support services provided in a residential setting to de-escalate and stabilize a mental health crisis.  Typically a 3-5 day stay during which care coordination, therapy, and supports are provided.  Anyone can refer, and transportation can be provided.
