Resources Heartland Family Service - Integrated Health Home

This "home" does not refer to a building.  It refers to a way of caring for your total health.  To join an Integrated Health Home there are three requirements:  live in Harrison, Mills, or Pottawattamie County; have Medicaid or be Medicaid eligible; have a mental illness that has an impact on your life.
